Patients experiencing symptoms of thrombosis should go to the closest Emergency Department for immediate investigation or assessment.
The VGH Thrombosis Clinic ONLY accepts urgent referrals for patients diagnosed with acute venous thromboembolism at the VGH Emergency Department, UBC Urgent Care and BC Cancer Vancouver Site. The clinic can only accept patients who are stable and can be managed safely in an outpatient setting.
Patients must also meet eligibility criteria outlined in the CERNER referral order.
To send an Acute VTE Treatment referral, fax the completed Acute VTE Treatment Referral Form for the referral site below to 604-875-5071. Please give the VGH Thrombosis Clinic Outpatient Treatment Program Information Sheet to the patient. This contains important contact information for our clinic and safety information.
Acute VTE Treatment Referral Forms
Patients from VGH Emergency Department or UBC Urgent Care will be called with an appointment for the next day. If the patient should be seen during the weekend or on a statutory holiday, the referring physician must speak with the VGH Hematologist on-call to arrange the appointment time.
Patients from BC Cancer Vancouver Site will be seen the following weekday. They should receive subcutaneous injection teaching and given low molecular weight heparin samples to continue until they are seen in the Thrombosis Clinic.
