Please include the reason for the referral, the patient\u2019s contact information and relevant laboratory results and imaging reports. Historical results and consultation notes from other specialists are very helpful. <\/p>\n\n\n\n<p>Our clinic assesses the urgency of all referrals based on the information provided. Inadequate information will delay the triage process and the referral will not be processed until relevant information is forwarded. Patients will be seen by the first available Hematologist, unless a specific Hematologist is requested.
