Please include all drugs, and also vitamins and supplements, as they can interfere with treatment. A medication list from your pharmacy will be useful.<\/li>\n\n\n\n<li><strong><font color=\"#c5252d\">A list of all your doctors and old medical records.<br><\/font><\/strong> Please bring the name and telephone number of all the doctors involved in your care.<\/li>\n<\/ul>\n\n\n\n<p>Where to Go for Your Appointment:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The Hematology Clinic and Thrombosis Clinic are located on the 6th floor of the Blackmore Pavilion.
