{"id":1712,"date":"2024-11-06T12:45:45","date_gmt":"2024-11-06T20:45:45","guid":{"rendered":"http:\/\/hematologybc.ca\/?page_id=1712"},"modified":"2024-12-04T22:50:16","modified_gmt":"2024-12-05T06:50:16","slug":"elective-referrals","status":"publish","type":"page","link":"https:\/\/hematologybc.ca\/?page_id=1712","title":{"rendered":"Elective Referrals"},"content":{"rendered":"\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>The VGH Thrombosis Clinic accepts elective non-urgent referrals for the following conditions:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Duration of anticoagulation<\/li>\n\n\n\n<li>Investigation for thrombophilia<\/li>\n\n\n\n<li>Primary or secondary thromboprophylaxis<\/li>\n\n\n\n<li>Venous thromboembolism in unusual site(s)<\/li>\n\n\n\n<li>Complicated anticoagulation management (e.g., cancer, APS, recurrent VTE)<\/li>\n\n\n\n<li>History of heparin-induced thrombocytopenia (HIT)<\/li>\n<\/ul>\n\n\n\n<p><strong>To send an elective referral, fax the completed the Elective Thrombosis Referral Form below, along with relevant laboratory and radiology reports, to 604-875-5071.<\/strong><\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link has-white-color has-text-color wp-element-button\" href=\"http:\/\/hematologybc.ca\/wp-content\/uploads\/2024\/11\/Form_Thrombosis_Elective_Referral.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Elective Thrombosis<\/a><\/div>\n<\/div>\n\n\n\n<p><br><strong>To facilitate the consultation, please provide the following information:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Bloodwork within past 3 months (CBC, INR, aPTT, creatinine, total bilirubin)<\/li>\n\n\n\n<li>Laboratory reports of previous thrombophilia testing<\/li>\n\n\n\n<li>Radiology reports of previous thrombotic events<\/li>\n<\/ul>\n\n\n\n<p>Once the referral form and all requested information are received, the referral will be triaged and prioritized accordingly. Incomplete referral forms will delay triaging and prolong wait times. The typical wait time for an elective referral is 3 \u2013 6 months.
